Genealogy Request Form

Terms of service

The Archives staff of the Diocese of Green Bay is happy to assist genealogists’ in their research needs. We are here to serve you.

After the patron agrees to the guidelines below, Archives staff will conduct the research. Staff will do its best to retrieve the records requested,  but cannot guarantee retrieval of any genealogy records.

Prior to proceeding with any request, we ask you to take a moment to read and understand the following terms of service. Thank you.

1. All patrons are asked to be aware of and consent to the following terms of service:

•  Requests are answered in the order received. The volume of requests and staff time are such that it will probably be two to three weeks before the request is completed.

•  Patrons may request additional searches, but subsequent requests will treated after earlier ones are completed and paid for.

•  The Archives does not have any general indexes. Researchers search church by church, sacrament by sacrament, year by year. Hence, the more specific the request (church and date), the better.

•  The Archives holds the records of baptism, marriage, and burial for most parishes.  The Archives does not hold birth certificates, marriage certificates, or death certificates. The burial record might mention the place of burial.  The Archives has very limited cemetery records which can help determine specific grave sites.

•  The Archives also has a limited collection of First Communion and/or confirmation of various parishes for certain years within the history of the given parish.

•  Archives records are closed to genealogical research after 1930. Staff will not be able to reproduce any of the records past December 31, 1930.

•  Genealogy records requested received via email, or over the phone or postal letter with a patron’s email address, will be transmitted by Archives staff via email. But, patrons should include their phone number and postal address for Archives record keeping and in case results need to be postal mailed.

•  Researchers may visit the Archives to perform personal research on Tuesdays (9 am-noon, 1 pm-4 pm) or by appointment. Private researchers contribute $10 for a half day or $15 for a whole day of research, in addition to a .25 printing fee for any records printed. Reservations may be confirmed by calling 920-272-8195.

•  Patron may request research for one or two unique ancestors.  Patron may request baptism, marriage, and or death (burial record) for each ancestor.  Complete the form as specifically as possible.  It is understandable that some fields might be left blank (such as the name of the church; if such is the case, answer not known).   For dates requested, indicate the range of time in which the church sacrament occurred or might have occurred.

•  Patron may use the notes field in the form to explain any special circumstances or pre-knowledge, such as variant name spellings or known burial places.

•  Note that this form is to request a record of a church sacrament and not a certificate of a church sacrament. The more specific and complete the submission is, the shorter will be the research turnaround time by Archives staff.

•  There is a set fee of $25.00 per research request plus .25 per copy/image. Staff will perform one hour of research for up to two ancestors’ sacramental records (e.g. baptism record search for ancestor #1, wedding and death record search for ancestor #2) Additional time for additional research subjects requires an additional payment  ($25 per hour). Prepayment is not required. Checks should be payable to Diocese of Green Bay, Inc. and mailed to:
